Florida Kings Hockey Club Commitment Letter 2022-2023 We, the undersigned participant and parent(s) and/or legal guardian(s) of said participant, by executing this Letter of Commitment, hereby commit to playing for the Florida Kings Hockey Club (“FLKHC”) from August 1, 2022 through March 31, 2023 (“FLKHC Season”) and further agree to the following terms and conditions: 1. General Terms. a. USA Hockey Membership. All participants are required to maintain and provide to FLKHC proof of USA Hockey Membership for the 2022-2023 season. b. Coaching. This commitment is to the FLKHC and not to a particular coach. The FLKHC will seek to provide the very best coaches, each individually background checked and qualified under then current USA Hockey rules, for each team and/or participant. c. Roster Additions. The FLKHC reserves the right to add players in any position to rosters, as it deems appropriate, in accordance with USA Hockey policies and procedures. d. Injuries. We recognize and accept that hockey is a dangerous sport that could result in injuries and/or long-term illness. In the unfortunate event of an injury, we agree to Hold Harmless the FLKHC and its Board of Directors, Coaching Staff, Advisors, and Volunteers. e. Maximum Participation. In accordance with the FLKHC mission to help each participant realize their complete potential, each participant commits to the following: i. Practices and Games. Barring injury or other extenuating circumstance, attend a minimum of 80% of practices and games. Failure to do so may result in USA Hockey, its regional governing organizations, and/or the FLKHC declaring the participant ineligible to participate in state, regional and/or national championship tournaments. ii. Homework. From time to time, Coaches may assign homework for participants. Such homework may include stick handling drills, physical fitness activities, stretches, reading, or video review. Every attempt should be made to complete these occasional assignments as they are an integral part of the physical and mental development of the participant. The FLKHC fully realizes and supports the notion that SCHOOL WORK shall take priority over any FLKHC related Homework in all instances. iii. Team Building Functions. The FLKHC will hold various team building functions throughout the year. These may include dinners, lunches, attending major sporting events, among other activities. Attendance at these functions is strongly recommended as they help to build comradery, team chemistry, and can even foster life-long friendships. f. Participation in additional Youth Hockey Programs. Participants may participate in additional Youth Hockey Programs and/or Events subject to the following terms: i. Prior Approval. The participant must gain the approval from their FLKHC Coach, the FLKHC Director of Hockey Operations, or the FLKHC Board of Directors. The purpose of this requirement is to ensure that such participation does not infringe upon the rules of USA Hockey and/or any related governing body. ii. Priority. We understand that if our player is registered with a non-FLKHC program that all FLKHC team events are to be prioritized. g. Release from FLKHC. Participants must seek and gain release from the FLKHC, not to be unreasonably withheld, prior to participating with any other USA Hockey affiliated youth hockey organization. Refunds will be granted in accordance with the refund policy outlined in Section 5 (“Payment Terms”) below. If applicable, participant will be required to return any equipment and/or materials belonging to the FLKHC prior to any release being granted. 2. Conduct. Participants, Parents and Spectators agree to abide by the following policies: a. FLKHC Codes of Excellence. The FLKHC has established a Code of Excellence for each Participants, Parents, and Spectators and have included those terms below. b. USA Hockey. Those rules, regulations, and codes of conduct as set forth by USA Hockey, and its related regional governing organizations. c. Home and Away Rinks. Any rules and regulations posted at any and all home and away rinks. 3. Discipline. Participants, Parents and/or Spectators who fail to comply with the applicable rules and regulations will be subject to Disciplinary Review before the FLKHC, USA Hockey, and/or a USA Hockey related regional governing organization. a. Outcomes. The outcome of Disciplinary Review may include no action, a warning, temporary suspension, or permanent dismissal from the program. b. Refunds for Suspensions or Dismissal. Under no circumstances will a participant be granted a refund or relief from any outstanding payments due as a result of a Suspension or Dismissal. c. Release as a result of Suspension or Dismissal. Under no circumstances will a participant be granted a release from the FLKHC as the result of a disciplinary based Suspension or Dismissal. 4. Tuition Schedule. Discount Options. The FLKHC offers several options for tuition discounts as described below. Note that the maximum discount allowable for any single participant will be 15%. a. Early Payment in Full. Any participant paying the total tuition amount prior to August 1, 2022 shall be entitled to a 5% discount. b. Discount for Multiple Participants from Same Household. Each additional participant from the same household shall be entitled to a 10% discount off the total tuition amount, to be applied equally to each scheduled payment. c. Discounts for Service Appreciation. Each participant with at least one parent that provides proof of qualification as one or more of the following shall be entitled to a 10% discount off the total tuition amount, to be applied equally to each scheduled payment: i. Active or Former Military Personnel ii. Active First Responder (Police Officer, Firefighter, Paramedic) 6. Payment Terms. a. Deposit. Each participant in the FLKHC Program agrees to pay a deposit of $600 within seven days of signing this letter. The FLKHC reserves the right to offer the participant’s position in the club to another participant if the deposit payment is not made within seven days. All deposit payments will apply toward the participant’s Tuition. b. Late Payments. Any scheduled payment not made within fifteen (15) calendar days of the Due Date may result in the suspension of that participant. Any suspended participant will be immediately reactivated once all scheduled payments have been made. In no event will participant suspension relieve the participant of their responsibility to make the remainder of the tuition payments. c. Final Payment. The final payment shall be due no later than December 15, 2022. Participants who have not made their final payment by December 15, 2022 may, in the sole discretion of the FLKHC, be suspended until such time that Final Payment has been made. d. Credit Card Payments. All payments made via credit card shall be subject to a 3.5% processing fee. e. Refunds. Refunds will be given in accordance with the following schedule: i. Prior to August 1, 2022. Full refund. ii. August 1st, 2022 or after. No refunds shall be granted and participant will be required to make all payments in accordance with the above schedule. FLKHC may, in its sole discretion, consider partial refunds in extenuating circumstances. 7. Included with FLKHC Program. The curriculum for the FLKHC Season includes: a. On-Ice Training. Typically conducted 3-4 days per week, 60 to 90 minutes per session. b. Office Ice Training. Typically conducted 2-3 days per week, 30 to 60 minutes per session. c. Power Edge Pro (“PEP”) Coaching. The FLKHC utilizes the world renowned PEP system to maximally develop each player. The PEP equipment, study materials, video tutorials, and additional coaching time are all included in and exclusive to the FLKHC program. d. Coaches Travel Costs. All travel costs for the FLKHC coaching staff is included. e. SAHOF Fees (Ages 10U and higher). If the FLKHC participates in the SAHOF program for the 2022-2023 season, all SAHOF related fees up to and including State Championships shall be paid for out of the Tuition. If the FLKHC does not participate in the SAHOF program for the 2022-2023 season, any unused monies will be used to cover additional tournament registration fees to provide a similar number (typically 16 to 18) of games played. f. Additional Tournaments. Tuition includes a minimum of 4 additional tournaments, tentatively 2 in South Florida, 1 in North Florida, and 1 out of state. This schedule is subject to change. Parents will be given ample advance notice of any required tournament participation. 8. Not Included with the FLKHC Program. The following items are not included in the FLKHC program. a. Mandatory Items. The following items are considered mandatory and must be purchased by each participant: i. Participant Equipment. Including Helmet, Skates, Gloves, Sticks and other related playing and safety equipment such as neck and mouth guards. Helmet, Gloves and Skates must be BLACK to match Jerseys. ii. Jerseys. Home and Away Jerseys, Practice Jersey, Socks, and Shell. iii. Warm Ups. Jacket, Pants, Jogging Shorts, and Off-Ice T-Shirts. iv. Equipment Bag. The official FLKHC Equipment Bag will be required for all participants at the 10U and higher levels. b. Optional Items. The FLKHC Team Store will provide participants and their families with the opportunity to purchase additional FLKHC logo gear including t-shirts, sweatshirts, hoodies, jackets, stickers, and more. c. Travel Costs. Each participant is required to pay for their own travel costs including, but not limited to, hotel, airfare, car rentals, food, tolls, etc. PARTICIPANT / PLAYER CODE OF EXCELLENCE As a player for the Florida Kings Hockey Club, I hereby agree to adhere to USA Hockey’s rules governing player participation as well as the following code of excellence: 1. There will be no swearing or abusive language on the bench, on the ice, anywhere within the rink, at any team function, online via social media or otherwise, or via text message. 2. There will be zero tolerance for lashing out at any official no matter what the call is. The coaching staff will handle all matters pertaining to officiating. 3. Anyone who receives a penalty will skate directly to the penalty box without interacting with the officiating staff, opposing players, or spectators. 4. Fighting will not be tolerated in any capacity, whether instigated or retaliatory. Any fighting will result in an appearance before the Discipline Committee. 5. There will be no drinking of alcoholic beverages, smoking, vaping, chewing of tobacco or use of illegal substance at any team function. 6. All players understand that participation in the Florida Kings Hockey Club and/or other USA Hockey sanctioned events is a privilege and they shall conduct themselves at all times in a manner that is befitting of such a privilege. 7. Any player that violates this Code of Excellence will be subject to further disciplinary action, up to and including, dismissal from the program. PARENT CODE OF EXCELLENCE 1. Do not force your children to participate in sports, but support their desires to play their chosen sports. Children are involved in organized sports for their enjoyment. Make it fun! 2. Encourage your child to play by the rules. Remember, children learn best by example, so applaud the good plays of both teams. 3. Try to avoid coaching your own child, whether at home or at the rink. Your advice, though well intentioned, may be counter to what the coaches are trying to teach at a particular moment. 4. Do not embarrass your child by yelling at players, coaches or officials. By showing a positive attitude toward the game and all of its participants, your child will benefit. 5. Emphasize skill development and practices and how they benefit your young athlete. De-emphasize games and competition in the lower age groups. 6. Know and study the rules of the game and support the officials on and off the ice. This approach will help in the development and support of the game. Any criticism of the officials only hurts the game, reduces the fun, and ultimately slows your child’s growth and development. 7. Applaud a good effort in both victory and defeat, and enforce the positive points of the game. Never yell or physically abuse your child after a game or practice – it is destructive. Work toward removing the physical and verbal abuse in youth sports, and de-escalate tense situations whenever possible. 8. Recognize the importance of volunteer coaches. They are important to the development of your child and the sport. Communicate with them and support them. 9. If you enjoy the game, learn all you can about hockey – and volunteer! The FLKHC is always looking for volunteers! SPECTATORS CODE OF EXCELLENCE 1. Display good sportsmanship. Always respect players, coaches and officials. 2. Act appropriately; do not taunt or disturb other fans; enjoy the game together. 3. Cheer good plays of all participants; and never boo opponents. Remember, these are just children and teenagers! 4. Cheer in a positive manner and encourage fair play; profanity and objectionable cheers or gestures are offensive and will not be tolerated. 5. Help provide a safe and fun environment; throwing any items on the surface can cause injury to players and officials. 6. Do not lean over or pound on the glass. This sets a bad example for the kids. 7. Support the referees and coaches by trusting their judgment and integrity. 8. Be responsible for your own safety - be alert to prevent accidents from flying pucks and other avoidable situations. 9. Respect locker rooms as private areas for players, coaches and officials. 10. Be supportive after the game - win or lose. 11. Recognize good effort, teamwork and sportsmanship.
